Vitrase Injections in Los Angeles
At Kare Plastic Surgery + Skin Health Center, the management of complex plastic surgery complications is a top priority. Our office is a tertiary referral center for patients who have undergone lip and facial fillers by other doctors and who are now unhappy with the results. Overfilling of hyaluronic acid facial fillers can lead to chronic swelling, an unappealing cosmetic appearance, puffiness, and disharmony of facial proportions.
Dr. Raffy Karamanoukian is a an expert revision plastic surgeon and can help you manage overfilling of the hyaluronic acid facial fillers Juvederm, Restylane, Voluma, Belotero, and Volbella. In fact, the injection of hyaluronidase Vitrase can help dissolve the filler. Enyzymes work by breaking down the molecular bonds that hold Juvederm and Restylane together as a filler. The injection of Vitrase or Hylenex works relatively quickly to break the molecular bonds and help the body resorb the fillers.
If you have Hyaluronic acid fillers by another doctor and are unhappy with the outcome of your procedure, you may be a candidate for Vitrase treatment in our Los Angeles and Beverly Hills plastic surgery practice. Please call our office for an appointment.

Tyndall Effect from Eyelid Fillers
The Tyndall effect is a known complication from lower eyelid fillers and is caused by injecting large amounts of an HA filler underneath thin skin. We see this problem most commonly in the lower eyelid tear trough when a bluish color appears underneath the skin. Some cases of the Tyndall Scattering can resolve on its own, but other times a Hyaluronidase Hylenex injection needs to me used to melt the filler. Our office specializes in the management of lower eyelid filler complications. We even see patients who have permanent fillers such as Bellafill or Artefill that was injected in the tear trough and lower eyelids.
What is Hyaluronidase?
Hyaluronidase is an enzyme that can melt HA fillers. Hylenex is a trademark brand of Hyaluronidase that is now used in the United States to dissolve fillers made with Hyaluronic acid. It is made of a safe protein that can break down and dissipate unwanted filler that is injected in the face and body. Several sessions are sometimes needed to help resolve the filler, depending on the strength of the Hylenex and the thickness and volume of the filler.
What can I expect after one session of Hylenex?
There are a lot of variables associated with how well your Hylenex injections will work on your injections. The volume of your filler and the strength of the Hylenex can affect how many sessions you are going to need to fully dissolve your filler. Dr. Karamanoukian may recommend partial dissolution of the filler so that you can still maintain some volume after the treatment. We can inject the Hylenex in most areas of the face and body.

Types of Lip Filler Hyaluronidase: Hylenex and Vitrase
There are several different types of hyaluronidase injections that are available in it United States, and many of these work in similar methods to dissolve hyaluronic acid fillers in the face or body. Hyaluronidase (Vitrase or Hylenex) is an enzyme that reverses hyaluronic acid facial fillers that are FDA approved for soft tissue augmentation. The dosing, interval timeline, and concentration of hyaluronidase injections can make a difference in the short and long-term outcome of filler reversal.
Technique is an important component of Hyaluronidase or Hylenex filler reversal. Dr. Raffy Karamanoukian at Kare Plastic Surgery in Santa Monica uses expertise in filler injections to determine the correct concentration and method of injection when dissolving a filler. It is important to determine, beforehand, the type of filler used and the amount of cross-linking within the Filler that may make it more or less responsive to hyaluronidase. Although filler reversal is commonly performed because of the cosmetic appearance of the lips or face, it can also be used in a more urgent situation when time is of essence.
There are several factors to consider when deciding whether or not to undergo a treatment with hyaluronidase. Some fillers are more resistant to this enzyme whereas others have less cross-linking and may be easier to dissolve. If you have more than one type of filler injected, there may be variable amounts of response to the Hyaluronidase.
If you've had the filler for a long period of time, this may suggest that your filler is highly cross-linked or permanent and may not respond as readily to the hyaluronidase injections. Dr. Karamanoukian is an expert in the removal of silicone biopolymers and other types of fillers from the face and body and can help you determine whether Hylenex is the most appropriate option for your case.
Inflammatory granulomas or nodularity from an injection may also be problematic and may not respond readily to hyaluronidase or highly next treatments. These may be removed with injections of corticosteroids or surgery depending on your specific need.
A typical Hyaluronidase treatment at Kare Plastic Surgery in Santa Monica takes about 30 minutes to perform and patients note mild discomfort with the injections. Topical or injection anesthetic can be used to improve comfort and patients often notice a response immediately with improvement for 12 to 24 hours thereafter. We would advise patients not to proceed with immediate filler Injections after Vitrase treatment.
